Choice/Transfer Request Forms
If you live within the Kelso School District boundaries, but would like you student to attend a different school in Kelso, complete an In-District Boundary Exception form.
If you do not live within the Kelso School District boundaries, but would like you student to attend a school in Kelso, complete a Choice Transfer Request form.
If you live within the Kelso School District boundaries, but would like to provide your student with home-based instruction, complete a Declaration of Intent to Provide Home-Based Instruction form. If you are providing home-based instruction but would like your student to remain enrolled part-time, also complete a Request for Part-Time Attendance form.
Behavioral Support Services
To prevent substance use and mental health disorders; when signs are present, to intervene early and appropriately; and when assessments indicate that treatment is required, to provide the right type of treatment, at the right place, by the right person/programs to help students achieve and maintain the highest quality of health and wellness.Behavioral Health Services (BHS) are available to Cowlitz County students seeking mental health and substance use treatment through the Great Rivers Behavioral Health Organizations Continuum of Care.
